Membership Requirements
The following is required to be included with the application:
Check list
1. Application
2. Completed Membership Payment Method
3. Complete application for the Membership Form
4. National Criminal Record Report.
5. Pastor or Supervisor recommendation (or an ordained minister of applicants who are a pastor)
6. Payment of $ 410.00 for registration fee (One time only) and $ 20.00 Monthly Fee ** ($ 430.00 Total)
7.-Updated copy of Personal resume.
8. List of Community Services Experiences
9. Copy of the Certificate of Completion of the Basic Chaplaincy Course
10. Letters of recommendation from two (2) business people
11. Copy of valid driver’s license
12. 2 passport-sized photos (clear enough to produce ID) or photos on the disk (or taken in class?)
13. Copy of the ministerial license and / or the Ordination (if the applicant is ordered or licensed by another organization)
14. Recommendation of a chaplain (useful if you know one)
15. Place and date of seminar they attended.
